The link below provides you an online format that will allow you to complete our credit application along with electronic signature, and to print/email a copy of the completed form for your records as well as ours. MIQ Logistics requires acknowledgement of our terms and conditions. Please note that our Credit Application requires this acknowledgement and allows for a copy to be printed for reference.

Carriers are subject to limited liability by law based on proof that they were responsible for loss. Cargo insurance is a critical component in your supply chain to ensure that you are covered for the entire loss amounts that may occur in a loss or damage scenario as well as a General Average claim. As such MIQ requires that attached insurance form be reviewed and acknowledged by our clients for your protection. The link below provides you an online format that will allow you to complete our Customs Power of Attorney along with electronic signature, print and email a copy of the completed form for your records as well as ours.
